Who we are

The Zebra Life is a nurse-owned business committed to assisting individuals with education and consultation services regarding rare and/or complex diseases. Integrity is our number one priority when it comes to your care.

Who we share your data with

We take your privacy seriously. No personal data is shared or sold to any third-party companies. Any information given during consultations are treated with strict HIPAA guidelines and nurse-patient confidentiality.

Medical Records

Medical Records are reviewed through Epic’s MyChart Share Everywhere platform. A unique code will be sent from you to allow limited, one-time access to your medical chart. This does not include identifiers, such as your social security number, and it does not show messages between you and your doctors or billing and insurance information. Date of birth is the only identifier associated with your name. The information shared include labs, medications, diagnoses and doctor visits, which is similar to what you see when you log into your MyChart. The access to your chart is revoked once the review is over. Documents and notes will be stored in the Simple Practice platform, which is a HIPAA compliant EMR software. Once our agreement is complete, your information is stored securely for seven years for liability reasons, then destroyed.

Cookies

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.

When you log in, cookies save your login information and your screen display choices. Login cookies last for two days, and screen option cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed.

What rights you have over your data

You may log-in to your Simple Practice portal at any time to see your information and session notes. You may request a digital copy of your entire file be sent to you via a secure, encrypted message. You have the right to request a correction or amendment to your notes if you believe information is inaccurate. We will review and respond to these requests promptly.You may terminate our consulting agreement at any time, effective immediately upon notice.
